Service Summary

The Social Work Department provide the following services to inpatients utilizing a case management approach:

• Consultation and assessment with multidisciplinary treatment teams, both in our hospital and the community.
• Discharge planning, which involves assessment of the patient and his/her social situation to determine appropriate post hospital care and facilitate placement, where needed.
• Facilitating connections for financial assistance, legal representation, educational upgrading and vocational rehabilitation, as necessary.
